Pregnancy Journal Prompts for Every Trimester

Thoughtful questions that change as your baby grows, from the secret hopes of the first trimester to the final countdown of the third.

Keeping a pregnancy journal is one of the most meaningful things you can do during these forty weeks. But staring at a blank page? That's hard. Especially when your body is exhausted and your mind is racing in twelve directions.

That's why we've put together prompts that meet you where you are. Each trimester brings different questions, because week 6 feels nothing like week 36.

First Trimester Prompts (Weeks 1–13)

The secret season. You might not be showing yet, but everything is already changing. These prompts honor the quiet, invisible work your body is doing.

Early Weeks (1–6)

  • How did you find out you were pregnant? Write down every detail, the time of day, who you told first, what you felt.
  • What are three words that describe how you feel right now?
  • What are you most hopeful about?
  • What scares you? (It's okay to write it down. The page can hold hard things.)
  • If you could send a message to your baby right now, what would it say?

Later First Trimester (7–13)

  • Describe your first ultrasound. What did you see? What did you feel?
  • Who have you told about your pregnancy, and how did they react?
  • What food cravings or aversions have surprised you?
  • What does your partner (or support person) do that makes you feel loved right now?
  • Write a letter to yourself at 6 months pregnant. What do you hope she knows?

Second Trimester Prompts (Weeks 14–27)

The golden stretch. Energy returns, the bump appears, and things start to feel real. This is when the physical and emotional worlds collide beautifully.

Mid-Pregnancy (14–20)

  • You felt your baby move. What was that moment like? Try to freeze it in words.
  • Describe your bump. What does it feel like to see your body change?
  • What names are floating around in your head? What do you love about each one?
  • What's one thing you want your child to know about who you are before you became a parent?
  • Take a bump photo. Describe what you see. (Don't judge, just notice.)

Anatomy Scan & Beyond (20–27)

  • The anatomy scan showed so much. What did you learn about your baby?
  • What traits do you hope your baby inherits from you? From your partner?
  • What's the best piece of parenting advice you've received so far?
  • Describe the nursery (or the dream of it). Colors, textures, the feeling you want the room to hold.
  • What song would you play for your baby right now? Why that one?

Third Trimester Prompts (Weeks 28–40)

The waiting room. Everything is heavy and close and real. These prompts are about making peace with the transition ahead.

Final Stretch (28–36)

  • What does your body need right now that you're not giving it? Rest? Water? Permission to slow down?
  • What are you most excited to teach your child?
  • Describe a moment from your own childhood that you want to recreate for your baby.
  • What kind of parent do you want to be? Write it down, not as a prescription, but as an intention.
  • Your baby can hear your voice now. What do you want them to know?

The Last Weeks (37–40)

  • Write a letter to your baby on the eve of their arrival. No filters. Just love.
  • What will you miss about being pregnant?
  • What are you ready to leave behind?
  • If your body could speak, what would it say to you right now?
  • One word for this journey. Just one. Write it big.
